AN ORDINANCE AUTHORIZING PAYMENT TO JOSHUA DAVIS, A CITY OF SPRINGFIELD POLICE OFFICER, TO SETTLE A WORKERS' COMPENSATION CLAIM FOR CLAIM NUMBER 16294G715500Joshua Davis was employed as a police officer for the City of Springfield on April 23, 2016 and reported an injury to his left arm and wrist when he fell while chasing a suspect. Mr. Davis seen in the emergency room for a fractured radius and underwent surgical repair in April of 2016; he was able to return to work full duty in July of 2016. Mr. Davis filed a workers' compensation claim ( claim number 16294G715500) and is willing to settle his claim in the amount of $30, 128.25 representing a permanent partial disability equivalent to 20% of an arm. Livingstone, Mueller, O'Brien and Davlin, P.C. recommend payment in the amount of $30, 128.25 to settle claim number 16294G715500 (IWCC case number pending).

This ordinance authorizes the execution of five property lease agreements for a one-year term for the Administrative Services Division for the Office of Public Utilities. The leased properties are: (1) 1351 Dickey Road (Site No. 94) Pawnee, Illinois, and comprises approximately 16.39 acres with a single-family residence; (2) 9820 Lost Forty Road (Site No. 98) Pawnee, Illinois, and comprises approximately 2 acres with a single-family residence; (3) 4120 KOA Road (Site No. 100) Rochester, Illinois, and comprises approximately 24 acres with a single-family residence and detached garage; (4) 4156 KOA Road (Site No. 80) Rochester, Illinois, and comprises approximately 2 acres with a single-family residence; and (5) 4188 KOA Road (Site 96), Rochester, Illinois, and comprises 1 acre with a single-family residence. The lease for Site 94 provides that the tenants will pay $650.00/month in rent. Last year’s rent was $650.00/month. The lease for Site 98 provides that the tenants will pay $900.00/month in rent for one year. Last year’s rent was $850.00/month. The lease for Site 100 provides that the tenants will pay $1,050.00/month in rent for one year. Last year’s rent was $1,000.00 per month. The lease for Site 80 provides that the tenants will pay $1,000.00/month in rent for one year. Last year’s rent was $950.00 per month. The lease for Site 96 provides that the tenants will pay $900.00/month in rent for one year. Last year’s rent was $850 per month. These are one-year leases.
