St. John's Hospital Springfield Illinois issued the following announcement on Nov. 17.
Certified application counselors will be available to help you enroll in a health coverage plan. Individuals and families have until December 15, 2018, to enroll in health coverage.
To enroll, it will help to bring the following:
• Full names and birth dates for any family member enrolling.
• Social Security Number for any family member enrolling.
• Proof of Illinois residency (Illinois driver’s license, state ID, etc.).
• Employer and income information (paycheck stubs, W-2 forms or other information about your family’s income).
