BALL-CHATHAM COMMUNITY UNIT SCHOOL DISTRICT 5: Attention Parents/Guardians of Upcoming Kindergarteners!

Ball-Chatham Community Unit School District 5 issued the following announcement on May 8.

If you did NOT pre-register your soon-to-be Kindergartener in February, please print and fill out the school registration documents, and drop them off to your assigned elementary school (in the building vestibule) on one of the days and times listed below. Or you may pick up printed copies (located in the building vestibule) on one of those dates to take home and complete, then bring back. Please note that a mask must be worn when entering the school. If you DID pre-register your child in February, nothing further needs to be done.

May 11, and May 18 from 7-11 a.m.

May 14, 21, 28 from 11 a.m. to 3 p.m.

In addition, we will also need residency documents, (items 1-3 below) as well as immunization records and a current physical if either of these are complete:

A certified copy of your child's birth certificate

A signed copy of your lease, mortgage or most recent tax bill

Two other pieces of mail with name and current address listed (such as utility bills, cable bill, etc.).

Any immunization records you may have (due by the first day of school in August)

Current physical (due by the first day of school in August)

The registration process will not be complete unless proof of residency is confirmed (either by a copy of a lease, mortgage, or most recent tax bill along with two other pieces of mail with name and current address listed).
